Our Team

The Arcturus Hospice Interdisciplinary team works together with you and your loved ones to develop a personalized plan of care based on your decisions and needs.

Patient and Family: Patient and their loved ones are an important part of our hospice team. We will work with you and your family to develop a specialized plan that meets your physical, medical, emotional, spiritual and social needs.

RN Case Manager: The Registered Nurse (RN) will provide skilled nursing visits, support and teaching to you and your caregiver and act as coordinator of your care with physicians and other members of the interdisciplinary team. The Case Manager will keep your doctor regularly informed of your condition.

On-Call Nurse: A Registered Nurse is available 24 hours a day to answer your questions and respond to any issues. If necessary, the on-call nurse will visit you to resolve any problem that might arise outside of normal hours.

Hospice Medical Director: The Hospice Medical Director attends hospice team meetings and provides advice and direction to the hospice team. The medical director works with your attending physician and Case Manager to ensure that the medical care you receive is closely monitored and meets your ongoing needs.

Primary (Attending) Physician: Your physician may continue to serve as your doctor and work with members of the Arcturus team including our physician to manage your care. Your physician will continue to prescribe medications for you and will be involved with approving any changes in your plan of care.

Hospice Nurse Practitioner: On occasion, the Hospice Medical Director may request that a hospice Nurse Practitioner visit you to provide recommendations of your care to the hospice team. These recommendations will be discussed with you as well as the interdisciplinary team.

Social Workers: Hospice Social Workers are available to you and your family to discuss the emotional challenges of critical illness. They may also assist you with problem-solving in a crisis, exploring financial needs, reviewing alternative living arrangements, funeral planning, healthcare decisions and utilizing other community resources.

Chaplain: Hospice chaplains are non-denominational clergy. They provide spiritual support as appropriate to your wishes, or will contact the spiritual support person of your choice, if requested. They may also assist with final arrangements as requested.

Certified Nurse Assistants: Hospice Home Health Aides provide personal care support such as bathing, grooming and dressing, as identified by your plan of care under the supervision of RN Case manager.

Hospice Volunteer: Arcturus Hospice Volunteers are an integral part of the Hospice team and essential to our organization. They help provide a variety of support services to patients and families such as companionship and running errands. Arcturus volunteers listen and provide comfort and assurance through their presence and personal caring.

Bereavement Coordinator: Bereavement counseling and support is provided to families and caregivers for at least one year after a loss. Support is provided through phone calls, visits, written materials about grieving, individual counseling and support groups.

Hospice Agency Leadership: The leadership team is comprised of the Director of Operations and Director of Clinical Services who serve to provide oversight of the program to ensure that every patient receives high quality hospice care and support
